Requesting an Absentee Ballot for the November 5 General Election

An absentee ballot request form for the November 5 General Election is located on the back page of this pamphlet. If you are already an on-going absentee voter, you do not need to complete this form. You will automatically receive a ballot for this election providing our office has your current residential/mailing address. Call our office if you have any questions about your absentee ballot status.

Complete the form for the following reasons only:

  • You will be out of town prior to election day. (General election ballots will begin arriving on October 21.)
  • You wish to become an ongoing absentee voter.
  • You wish to use this form to change your voter registration address. You must be a registered voter and moving within Thurston County to use this form. (Please be sure to check the “New Permanent Address” box or complete the change of address form on page 17.)

For Military Members, Overseas Citizens, and Hospitalized Voters
