SELF-HELP CENTER - FORM PACKETS

The use of specific domestic relations forms is mandated by the Washington State Legislature.  All parties in family law cases must use these forms.  Forms may be purchased from the Clerk's Office individually for a cost of 50 cents per page or in form packets that range from $5.00 to $40.00.  The pro se litigant may find it helpful to seek the Family Court Facilitator's assistance when selecting forms, reviewing completed forms and answering questions regarding forms, court procedures or community resources.

Prior to coming to the Clerk's Office, you may select the form packet you need by accessing our On-Line Self-Help Center below.   Read the first two pages to determine which instructions fit your situation.  Once you have found the packet that best fits your situation, WRITE THE PACKET NUMBER DOWN.  Come to the Clerk's Office at the Family and Juvenile Court and request that packet number.  You may also purchase packets by mail.  Send your check or money order for the proper amount and a self-addressed stamped envelope along with your request.  Remember, packets are non-refundable so be sure you have selected the proper packet number. 

Self-help, resources, forms and instructions are also available through the Administrative Office of the Courts and the Northwest Justice Project.  If you are unsure what forms to download, you are strongly urged to purchase a Form Packet so you have all the forms required for your case.
