Earthquake Damage Report Form

To help County Officials accurately assess damages from the recent earthquake, residents of unincorporated Thurston County are urged to fill in the following form.  When complete, click the "Send Information" button.  The information will be sent to Thurston County Division of Emergency Management.  A county building inspector will visit your home to perform an earthquake damage assessment.

NOTE:  If you have earthquake damage, you may be eligible for disaster assistance from the Federal Emergency Management Agency (FEMA).  To register for financial assistance, you must also call FEMA at 1-800-462-9029.
