The projects listed below are the main areas of partnership with health care providers to achieve more chronic disease prevention in Thurston County:

  • LiveWell – CDSM - complements the efforts of medical professionals by expanding the availability of chronic disease self-management to employed adults.  ...more
  • Diabetes Collaborative – local planned care initiative prepares physicians to serve chronically ill patients better
  • Motivational Interviewing – improving physician skills in communicating with patients to motivate healthy behavior changes
  • Healthy Child Weight – increasing knowledge of best practices in childhood obesity prevention and developing new interventions.

Diabetes Collaborative

A local adaptation of the Washington State Collaborative, this project involves physicians by setting up chronic disease electronic medical tracking systems and holding quarterly learning sessions related to prevention and treatment of diabetic patients.  Physicians of Southwest Washington leads this project with funding from a Thurston County grant, and is currently pursuing additional support to sustain and continue to expand the Collaborative for local physicians.

Motivational Interviewing

Led by Physicians of Southwest Washington, this component of the ASIST2010 project helps physicians learn more about communicating with their patients in a way that supports personal motivation and self-care to improve patient quality of life in the face of chronic disease.  Presentations are made each year to reach new physicians with this important technique.

Healthy Child Weight

This project aims to encourage local health care providers who serve child patients family practice physicians and pediatricians - to use best practices in childhood obesity prevention.  A workgroup provides advice, and a series of discussions with local providers during mid-2008 culminated with the Healthy Child Weight Forum in September of 2008.  The HCW Forum event featured a presentation by childhood obesity researcher and pediatrician Dr. Lenna L. Liu of University of Washington and Seattle Children’s Hospital.
