We would like to let you know about the following resources for your health:

 

Family Planning 

Take Charge is the family planning benefits program that many clients have enrolled in at our clinic.  It covers most of the charges associated with your family planning services.  Other clinics that can provide Take Charge coverage are:  Planned Parenthood Olympia Health Center at 1-800-230-PLAN, Eastside Women’s Health Clinic at 943-5127, and Yelm Wellness Center at 458-5438.  For more information about Take Charge see http://hrsa.dshs.wa.gov/FamilyPlan/Take Charge/TC.index.htm or call 1-800-562-3022. Plan B is available over the counter for individuals 18 and older and available by prescription for women under 18.  If you need a prescription for insurance coverage, please ask your provider.  To find out more about Plan B, go to http://ec.princeton.edu/

Sexually Transmitted Infections/HIV

If you receive medical care from a primary care provider, you may ask for STD/HIV screening as part of your routine care.  You may qualify for other state health care programs, call or visit your local community service office to find out.  If you need assistance locating your community service office call the toll free hotline at 1-800-770-4334. Anonymous HIV testing and counseling is available at UCAN.  For more information, call 352-2375.

General Health/Women's Health

You may qualify for a free women’s health exam, including mammogram if you:  Are age 40 to 64, are on a limited income, have no or limited insurance.  For more information on the Washington Breast & Cervical Health Program, call 1-800-992-1817.  For other languages, toll free 1-888-202-3301.

CHOICE Regional Health Network provides help with enrollment in affordable health insurance for low income individuals and families.  They also assist with Breast & Cervical Health Program enrollments.  For more information about CHOICE services, call 493-4550 or 1-800-981-2123. For Spanish line, call 1-888-493-8397.

How do I get a copy of my health records?

If you want a copy of your health records, please note the following options:

  • Contact your health care provider.  With your permission, your health care provider can request a copy of your records from us.

  • You may visit our health department to request a copy of your records.  In order to receive a copy of your health records, you will be asked to sign a records release form and show proper photo IDPlease Note:  Records cannot be release without appropriate identification.
