What is LiveWell?

The LiveWell Workshop series is based on Stanford University’s Chronic Disease Self-Management Program (CDSMP). The LiveWell workshops are offered by Thurston County Public Health and Social Services and Physicians of Southwest Washington under a licensed agreement with the developers of the program at Stanford University’s Patient Education Research Center. Sessions are highly participative, where mutual support and success builds the participants’ confidence in their ability to manage their health and maintain active lives. Workshop sessions are facilitated by two Stanford-trained leaders, at least one having personal experience with a chronic health condition. These workshops are gender-focused with 2.5 hour sessions being held once a week for six weeks.

Are These Workshops For Me?

Thurston County Public Health and Social Services and Physicians of Southwest Washington invite you to participate if:

  • You are an employed adult in Thurston County.
  • You have diabetes or another chronic health condition (heart disease, cancer, arthritis, MS, depression, etc.).

What Will I Learn?

Among the topics included in every workshop are:  Action Planning, Physical Activity, Pain/Fatigue Management, How to Plan for Your Future, Healthy Eating, Medication Usage, Making Informed Decisions, and How to Work with Your Healthcare System.

Where Are The Workshops and How Do I Sign Up?
