What is LiveWell?

LiveWell workshops are based on Stanford University’s Chronic Disease Self-Management Program (CDSMP). LiveWell workshops were offered by Thurston County Public Health and Social Services and Physicians of Southwest Washington between October 2008 and August 2010 under a licensed agreement with the developers of the program at Stanford University’s Patient Education Research Center.

CDSMP workshops are 2.5 hour sessions held once a week for six weeks. Sessions are highly participative, where mutual support and success builds the participants’ confidence in their ability to manage their health and maintain active lives. Workshop sessions are facilitated by two Stanford-trained leaders, at least one having personal experience with a chronic health condition.

Coordinating Your Own CDSMP

What Will I Learn?

Among the topics included in every six week workshop series are: Action Planning, Problem Solving, Physical Activity, Pain/Fatigue Management, How to Plan for Your Future, Healthy Eating, Medication Management, Making Informed Decisions, How to Work with Your Healthcare Provider and System and many other topics. 

Are These Workshops For Me?

CDSMP workshops are for you if you are living with a chronic health condition such as arthritis, chronic pain, depression, diabetes, heart disease, cancer, MS, and many others.
